ENROLMENT PROCEDURES
1. INQUIRE AT THE ADMISSION OFFICE
2. SETTLE THE APPLICATION FEE AT THE CASHIER
3. GET THE EXAMINATION SCHEDULE
4. SECURE FROM THE REGISTRAR/ADMISSION OFFICER YOUR STUDENT’S FOLDER WITH FORMS TO BE FILLED-UP BY THE PARENTS
- Applicant’s Data Sheet
- Parent Interview Form
- Medical/Health Form
5. BEFORE TAKING THE ENTRANCE EXAM, SUBMIT THE FOLLOWING TO THE ADMISSION OFFICER/REGISTRAR
- Photocopy of birth certificate
- Photocopy of baptismal certificate (for R. Catholics)
- Form-138 (Report Card) (transferee)
- Certificate of Good Moral Character (Gr. IV-VI) (transferee)
- Recommendation Letter (Elementary & High School). It should bear the official school seal and enclosed in a sealed envelope.
- 6 pcs. 1 x 1 recent photo of the child (blue background)
- Medical Certificate from Family Doctor
6. PARENT AND CHILD INTERVIEW. For Casa applicants, the result will be discussed right after the evaluation. However, for elementary applicants, the examination result will be discussed after the submission of a Report Card, Certificate of Good Moral Character, and Recommendation Letter.
7. PAY THE RESERVATION FEE &/OR ENROLMENT FEE
8. FILL-UP REGISTRATION FORM
9. SECURE THE FOLLOWING:
- Handbook/Diary
- Uniforms
- Books
- Notebooks
- 2 pcs. 1 x 1 recent photos of mother, father, and guardian (picture of those who are authorized to fetch the child to/from school if non-bus riders)
- Fetcher’s ID (for non-bus riders)
- Commuter’s ID (Parents should submit a letter allowing their child to commute)
- Letter of intention and house sketch for school bus riders
- Request for 137 – for Transferee
